Intern killed by lion due to large ball preventing gate closing

An investigation has revealed that a gate blocked by a ball at a North Carolina animal preserve allowed a lion to reach three people at the start of a fatal attack in December. Alexandra Black, 22, had been working at the Conservators Center in Burlington, N.C., as an intern for approximately two weeks at the time of her death. She was killed after the lion got loose from the enclosure.

With only 2 weeks experience how could you expect her to be on the ball when it comes to this kind of thing?
